Cook pasta in salted water according to package directions until al dente. Reserve ½ cup pasta water, then drain.
While the pasta cooks, heat olive oil in a large skillet over medium heat. Add garlic and sauté for 1-2 minutes until fragrant.
Add cream, lemon zest, and lemon juice. Stir to combine and bring to a gentle simmer.
Add cooked pasta and Parmesan to the sauce. Toss to coat, adding reserved pasta water a little at a time until desired consistency is reached.
Season with salt and pepper to taste. Garnish with chopped basil or parsley. Serve immediately.
